When can you change your last name
In accordance with the legislation of Ukraine, a person's name consists of a surname, a proper name and a patronymic. Therefore, from a legal point of view, a person's surname changes as part of the name change procedure.
In Ukraine, a person has the right to change his surname in several cases. First, it happens when a marriage is contracted or dissolved. So, a person has the right to get the spouse's surname, or vice versa, to get back the maiden name. A person's surname can be changed in the event of his adoption. In addition, for children under 7 years of age, it is compulsory to change the surname in the event of a change of parents.
Where to apply for a change of surname
Issues related to the change of surname are within the competence of the registry office at the place of residence of the person. If a person permanently resides outside Ukraine, then to change the surname, you should contact the embassy or consulate.
To change the surname, an adult must provide the following documents:
- application;
- passport;
- birth certificate;
- certificate of marriage or its dissolution;
- birth certificate of minor or minor children (if any);
- certificate of change in the name of the applicant, his father or mother (if this has already happened before);
- a photograph;
- a receipt for payment of the state duty.
If the surname of a person between the ages of 14 and 16 changes, the following documents are submitted:
- birth certificate;
- a certificate from the housing office about the place of residence;
- written parental consent to change the surname;
- a receipt for payment of the state duty.
When changing the surname by a person who has reached the age of 16, but who does not yet have a passport, the following are provided:
- birth certificate;
- a certificate from the housing office about the place of residence;
- a receipt for payment of the state duty.
What is the amount of the state duty for changing the surname
For changing the surname, a state duty is charged in the amount of 0.3 non-taxable minimum incomes of citizens. In monetary terms, it is 5 hryvnia 10 kopecks. When you change your surname again, the amount of the state duty is already 3 non-taxable minimum incomes of citizens (51 hryvnia). The exception is cases when the change of surname is due to the registration of marriage. Here the state duty is not paid.
How long does it take to change your surname
An application for changing the name with the documents attached to it is considered by the registry office within 3 months. Over this time
the militia prepares its opinion on the possibility of changing the surname After that, the person is issued permission to change the surname. Having received it, you must, within three months, apply to the registry office for state registration of the change of surname.
Changing the surname requires the re-registration of all documents that relate to the person's identity. These include, in particular: passport, driver's license, certificate of assignment of an identification code, etc.
