A statement about a crime or an administrative offense is submitted to the duty department of the internal affairs body, where it will be registered and an appropriate check will be carried out. Police officers are required to accept such a statement even if the event took place in another area.

Step 1
There are no clear requirements for an application to the internal affairs bodies, so fill it out in any form by hand or type it on a computer.
Start by specifying the details. In the upper right corner, write the following data: "To the head of the Internal Affairs Directorate of the Ministry of Internal Affairs of the Russian Federation according to _ (hereinafter, the name of the internal affairs body, rank and full name of the official)." After that, state your data (name, place of residence, place of work, contact phone number).
Step 2
If you want to receive a message about the results of the check carried out on your application not at your place of residence (for example, you are leaving on a long business trip), then for feedback, indicate the required address ("address for correspondence: _" or "please send your answer to _") …
Step 3
Title this document as "Statement". In the main text with a red line, describe what, where, when and under what circumstances happened, indicating the specific meta and time. For example, “2011-27-09 at about 23.00 I was returning home on the street. Kirov g. N. Passing the shop "Products", located in the house number 34 on the specified street, from the back I felt a blunt object hit in the head area. After the impact, I lost consciousness. When I woke up at about 00.00, I discovered the loss of my belongings: a cell phone and a watch."
Step 4
In the next part of the statement, state your request addressed to the head of the law enforcement agency: “On the basis of the foregoing and guided by Art. 141 of the Criminal Procedure Code of the Russian Federation (or clause 3 of Article 28.1 of the Administrative Code of the Russian Federation, if it is not a crime, but an offense), I ask you to initiate a criminal case on the fact that unknown persons committed an attack on me."
Step 5
If necessary, include in the text of the statement information about personal injury, moral damage or the amount of real damage, as well as the data of witnesses or eyewitnesses, if any. Don't forget to sign and date the statement.
Step 6
Based on the results of verification measures, a criminal case will be initiated on your application or a decision will be issued to refuse to initiate it. Also, your application can be transferred to jurisdiction.
